POSITION SUMMARY

As an Adult Therapist at Polara Health, you will work
collaboratively with a diverse team of mental health professionals
to provide individual and group therapy services to adult clients.
You will conduct assessments, develop treatment plans, and
implement evidence-based therapeutic interventions to support
clients in achieving their mental health goals.
ESSENTIAL RESPONSIBILITIES
Conduct comprehensive assessments of adult clients to determine
their mental health needs and develop appropriate treatment
plans.
Provide individual therapy sessions using evidence-based
therapeutic modalities, tailoring interventions to meet the unique
needs of each client.
Facilitate therapeutic groups to promote skill-building, coping
strategies, and peer support.
Collaborate with the interdisciplinary team to ensure coordinated
and holistic care for clients.
Monitor and evaluate clients' progress, adjusting treatment plans
as needed and documenting outcomes.
Provide crisis intervention and support to clients in acute
distress.
Educate clients and their families about mental health conditions,
treatment options, and community resources.
Maintain accurate and timely clinical documentation in compliance
with regulatory standards and best practices.
Stay up-to-date with the latest research and therapeutic approaches
in the field of mental health.
Provides individual and group therapy services as allowed by
credentialing.
Works internally with clinical and medical teams ensuring
coordination of care and comprehensive service and treatment
planning.
Provides outreach and engagement to clients enrolled with Polara
Health as needed and as time allows.
Works closely with local community partners and forges close
relationships to ensure on-going collaboration and coordination of
care surrounding behavioral health services.
Participates as a team member and attends regular staffings and
supervision.
Provides documentation and requests for authorization as required
by funding sources.
Coordinates and assists treatment team in treatment planning and
continuum of care.
Maintains clinical records per agency policy and procedure.
Attends team meetings, case reviews, staff meetings, supervisory
sessions, and in-service trainings as scheduled.
Manages all aspects of treatment, including referrals to additional
resources as appropriate.
Assists in department strategic planning and implementation through
participation in department staffings, staff meetings, training,
and other meetings as assigned.
Actively participates in agency affairs, including marketing,
promotion of chemical dependency services, development of programs,
and other related duties as assigned.
Participates in training, workshops, seminars, both internal and
external, as required by agency policy and procedure and
certification requirements.
Participates in the Residential Unit on call system.
Conducts necessary case management to ensure proper referral and
follow-up on associated services.
Documents and reports to referral sources, both external and
internal.
Performs other duties as assigned.
QUALIFICATIONS
- 21 years of age or older; or minimum 18 years old and licensed or
certified under A.R.S. Title 32 and providing services within the
personnel member's scope of practice; or minimum 18 years old for
select administrative positions
- Must be able to provide or obtain an AZDPS Level One Fingerprint
Clearance Card
- Must possess a valid Driver License
- Master's degree in Counseling, Psychology, Social Work, or a
related field.
- Current licensure or eligibility for licensure as a Licensed
Professional Counselor (LPC), Licensed Clinical Social Worker
(LCSW), or similar license in the state of Arizona.
- Experience working with clients in a mental health setting is
preferred.
- Strong knowledge of evidence-based therapeutic approaches for
treating mental health disorders.
- Ability to establish rapport and build therapeutic alliances with
diverse populations.
- Excellent communication and interpersonal skills.
- Empathy, compassion, and a non-judgmental approach to client
care.
- Familiarity with electronic health records (EHR) systems and
basic computer skills.
- Commitment to maintaining the highest ethical and professional
standards.
