Job Title: Registered Nurse --- Care Manager, Utilization Review Job Specialty: Care Manager, Utilization Review RN Job Duration: 13 Weeks Shift: Monday to Friday, 8am-5pm with weekend on-call rotation (1 weekend a month) Guaranteed Hours: Not specified Experience: Minimum of 3 years of clinical nursing experience, with at least 2 years on a medical-surgical unit, PACU, or higher acuity inpatient unit (ICU, Step-Down, Telemetry, etc.) or home health (direct care). Must have 1 year of traveling experience as a contractor. License: Valid, current, full, and unrestricted Professional Nursing License to practice as a Registered Nurse in any state, Commonwealth, or territory. Certifications: Must maintain current Basic Life Support (BLS) certification by the American Heart Association (AHA) throughout employment. Must-Have: - Fingerprinting to be submitted and cleared prior to start. - Current immunizations, negative TB skin test proof from the last two years, documentation of 2 MMR vaccinations, Tdap, and Flu shot. - Clean background check and negative 10-panel drug screen within 7 days prior to arrival. Job Description: - Serve as an Outpatient/Inpatient Case Manager/Utilization Review Registered Nurse responsible for direct supervision and supportive contact for specific patient assignments and high-risk patients. - Design and manage a continuum of care focusing on empowering clients to achieve demonstrable outcomes and self-sufficiency. - Perform Utilization Review and Management/Clinical Documentation Chart Reviews of patients in the acute clinical care and ambulatory care settings. - Collaborate with the Social Work Case Manager on Discharge Planning and Care Transitions. - Work closely with the Physician Advisor and participate in the Utilization Review Committee, conducting Clinical Documentation Chart Reviews related to peer review, financial or insurance determination, or claims denials. - Link clients with Community Social Service providers, health care providers, substance abuse, and mental health providers to achieve specific goals. - Experience working with culturally diverse low-income populations facing barriers to self-sufficiency, such as illiteracy, welfare dependency, domestic violence, substance abuse, and mental health issues. - Commitment to Case Management Services and collaboration with Purchased and Referred Care Case Specialist. - Orientation every two weeks. - COVID vaccinations required (no religious exemptions accepted; medical exemptions require approval).
